BMC Cancer dergisinde yay\u0131mlanan bu yenilik\u00e7i ara\u015ft\u0131rma, ultrasonografinin (US) manyetik rezonans g\u00f6r\u00fcnt\u00fcleme (MRI) ile kar\u015f\u0131la\u015ft\u0131r\u0131ld\u0131\u011f\u0131nda te\u015fhis ba\u015far\u0131s\u0131n\u0131 inceleyerek, ameliyat sonras\u0131 takipte yeni bak\u0131\u015f a\u00e7\u0131lar\u0131 olu\u015fturuyor. \u00c7al\u0131\u015fma, osteosarkom n\u00fcks\u00fcn\u00fcn ultrasonografik \u00f6zelliklerini mercek alt\u0131na al\u0131rken, mevcut takip protokollerini sorgulay\u0131p ultrasonografinin g\u00fc\u00e7l\u00fc bir alternatif olabilece\u011fini iddia ediyor.<\/p>\n<p>Primer kemik t\u00fcm\u00f6rleri, \u00f6zellikle osteosarkom, tedavi sonras\u0131 lokal n\u00fckslerin erken tespitiyle hastalar\u0131n prognozunu do\u011frudan etkileyen \u00f6nemli bir t\u0131bbi alan olarak \u00f6n plana \u00e7\u0131k\u0131yor. S\u00f6z konusu ara\u015ft\u0131rmada, yedi y\u0131ll\u0131k retrospektif klinik veriler \u00fczerinden US ve MRI\u2019n\u0131n ger\u00e7ek hayat ko\u015fullar\u0131ndaki tan\u0131sal etkinli\u011fi kar\u015f\u0131la\u015ft\u0131r\u0131lm\u0131\u015ft\u0131r. Bu kapsaml\u0131 analiz, takip protokollerinin geli\u015ftirilmesi ad\u0131na paha bi\u00e7ilmez bulgular sunuyor.<\/p>\n<p>Ara\u015ft\u0131rman\u0131n en \u00e7arp\u0131c\u0131 bulgusu, lokal yumu\u015fak doku n\u00fckslerinin tespitinde US ve MRI aras\u0131nda duyarl\u0131l\u0131k, \u00f6zg\u00fcll\u00fck ve genel do\u011fruluk a\u00e7\u0131s\u0131ndan istatistiksel anlaml\u0131 fark\u0131n olmamas\u0131d\u0131r. Buna ra\u011fmen, cerrahi sahadaki yumu\u015fak dokulardaki morfolojik de\u011fi\u015fiklikleri saptamadaki etkinli\u011fi olduk\u00e7a y\u00fcksek. Ara\u015ft\u0131rma, osteosarkom n\u00fckslerinde ultrasonografik belirgin \u00f6zelliklere dikkat \u00e7ekmekte ve \u00f6zellikle t\u00fcm\u00f6r\u00fcn boyutu ile anatomik yerle\u015fimin n\u00fcks tespitinde belirleyici parametreler oldu\u011funu ortaya koymaktad\u0131r. Bu durum, ultrasonografinin sadece n\u00fcks\u00fc tespit etmekle kalmay\u0131p, ayn\u0131 zamanda t\u00fcm\u00f6r dinamiklerinin yerinde analizine imk\u00e2n tan\u0131d\u0131\u011f\u0131n\u0131 g\u00f6stermektedir.<\/p>\n<p>\u00c7al\u0131\u015fman\u0131n geli\u015ftirdi\u011fi sonografik parametreler \u00fczerine in\u015fa edilen tan\u0131 modeli, alan alt\u0131ndaki e\u011fri (ROC) analizinde 0,973 gibi m\u00fckemmele yak\u0131n bir skor elde ederek, lokal yumu\u015fak doku n\u00fcks\u00fcn\u00fcn varl\u0131\u011f\u0131 ile yoklu\u011funu ay\u0131rt etme kabiliyetinin ne denli y\u00fcksek oldu\u011funu g\u00f6stermi\u015ftir. Klinik pratikte bu de\u011fer, US\u2019un tek ba\u015f\u0131na veya MRI ile birlikte kullan\u0131ld\u0131\u011f\u0131nda y\u00fcksek g\u00fcvenilirlik sundu\u011funun en \u00f6nemli g\u00f6stergesidir.<\/p>\n<p>Ultrasonun 96,6% olarak bildirilen duyarl\u0131l\u0131k oran\u0131, yanl\u0131\u015f negatif vakalar\u0131n olduk\u00e7a az oldu\u011funu g\u00f6stererek tan\u0131 atlamas\u0131n\u0131 minimize etmekte, 90,9% \u00f6zg\u00fcll\u00fck ise gereksiz giri\u015fimlere ve hastan\u0131n psikolojik y\u00fck\u00fcne yol a\u00e7an yanl\u0131\u015f pozitiflerin d\u00fc\u015f\u00fck seyretti\u011fini teminat alt\u0131na almaktad\u0131r. Toplamda %94,6 do\u011fruluk oran\u0131, US&#8217;un lokal yumu\u015fak doku n\u00fcks\u00fc izlemedeki g\u00fcvenilirli\u011fi ve tutarl\u0131l\u0131\u011f\u0131 konusunda g\u00fc\u00e7l\u00fc kan\u0131t sa\u011flam\u0131\u015ft\u0131r.<\/p>\n<p>Pozitif prediktif de\u011fer (PPV) %95 ve negatif prediktif de\u011fer (NPV) %93,8 olarak a\u00e7\u0131klanmas\u0131, ultrasonun hem n\u00fcks varl\u0131\u011f\u0131nda hem yoklu\u011funda hastaya sunulan kararlarda y\u00fcksek do\u011fruluk sundu\u011funu g\u00f6stermekte ve klinik karar destek s\u00fcre\u00e7lerinde pratik bir avantaj yaratmaktad\u0131r. Bu parametreler, US\u2019un hem tan\u0131y\u0131 koymada hem de tan\u0131y\u0131 d\u0131\u015flamada g\u00fc\u00e7l\u00fc bir imk\u00e2n oldu\u011funa dikkat \u00e7ekmektedir.<\/p>\n<p>Stratejik a\u00e7\u0131dan bak\u0131ld\u0131\u011f\u0131nda, ultrasonografinin postoperatif takip protokollerine entegrasyonu ekonomik ve lojistik avantajlar sunmaktad\u0131r. Ultrason cihazlar\u0131 MRI ile kar\u015f\u0131la\u015ft\u0131r\u0131ld\u0131\u011f\u0131nda daha yayg\u0131n, d\u00fc\u015f\u00fck maliyetli ve ta\u015f\u0131nabilir olmas\u0131n\u0131n yan\u0131nda, ger\u00e7ek zamanl\u0131 dinamik g\u00f6r\u00fcnt\u00fcleme imk\u00e2n\u0131 sa\u011flamas\u0131, klinisyene anl\u0131k de\u011ferlendirme ve h\u0131zl\u0131 m\u00fcdahale yapma \u015fans\u0131 vermektedir. Bu \u00f6zellikler, primer kemik t\u00fcm\u00f6r\u00fc hastalar\u0131nda erken n\u00fcks tespiti ve m\u00fcdahale a\u00e7\u0131s\u0131ndan hayati \u00f6nem ta\u015f\u0131maktad\u0131r.<\/p>\n<p>Bununla birlikte, ara\u015ft\u0131rma sonografinin efektif kullan\u0131m\u0131n\u0131n operat\u00f6r deneyimine ba\u011fl\u0131 oldu\u011funu vurgulamaktad\u0131r. Y\u00fcksek duyarl\u0131l\u0131k, \u00f6zg\u00fcll\u00fck ve eri\u015fim kolayl\u0131\u011f\u0131 gibi avantajlar\u0131 ile ultrason, hasta bak\u0131m kalitesini art\u0131racak, k\u00fcresel anlamda eri\u015filebilirli\u011fi y\u00fckseltecek ve onkoloji takip protokollerine y\u00f6n verecek \u00f6nemli bir y\u00f6ntem olarak \u00f6n plana \u00e7\u0131kmaktad\u0131r.
