Yak\u0131n zamanda Avrupa merkezli \u00e7ok merkezli SECRETO \u00e7al\u0131\u015fmas\u0131ndan elde edilen yeni veriler, \u00f6zellikle patent foramen ovale (PFO) ta\u015f\u0131yan gen\u00e7lerde migrenle birlikte kanser, karaci\u011fer hastal\u0131klar\u0131 gibi nontradisyonel fakt\u00f6rlerin inme riskini belirgin bi\u00e7imde art\u0131rd\u0131\u011f\u0131n\u0131 ortaya koydu. Bu bulgular, gen\u00e7 eri\u015fkinlerde kriptogenik inmeye yakla\u015f\u0131mda kapsaml\u0131 risk de\u011ferlendirmesinin gereklili\u011fini vurguluyor.<\/p>\n<p>Patent foramen ovale, do\u011fumdan sonra kapanmas\u0131 gereken kalbin \u00fcst odac\u0131klar\u0131 aras\u0131ndaki k\u00fc\u00e7\u00fck deli\u011fin %25 vakada a\u00e7\u0131k kalmas\u0131 durumudur. Bu anatomik durum \u00e7o\u011funlukla herhangi bir belirti vermezken, kriptogenik inmelerle olan ili\u015fkisi uzun zamand\u0131r ara\u015ft\u0131r\u0131lmaktad\u0131r. SECRETO \u00e7al\u0131\u015fmas\u0131, PFO varl\u0131\u011f\u0131 ve yoklu\u011fu ile inme risk fakt\u00f6rlerinin birebir kar\u015f\u0131la\u015ft\u0131rmas\u0131n\u0131 yaparak kalp anomalisi varl\u0131\u011f\u0131nda hangi fakt\u00f6rlerin \u00f6ne \u00e7\u0131kt\u0131\u011f\u0131n\u0131 detayland\u0131rd\u0131.<\/p>\n<p>Ara\u015ft\u0131rmada 18\u201349 ya\u015f aras\u0131 toplam 1000\u2019den fazla ki\u015fi analiz edildi; yar\u0131s\u0131ndan fazlas\u0131 inme ge\u00e7irmi\u015f, di\u011fer yar\u0131s\u0131 ise kar\u015f\u0131la\u015ft\u0131rma grubu olarak de\u011ferlendirildi. Ya\u015f ortalamas\u0131 41 olup kat\u0131l\u0131mc\u0131lar\u0131n neredeyse yar\u0131s\u0131 kad\u0131nd\u0131. \u00c7al\u0131\u015fmada hipertansiyon, sigara, obezite gibi 12 geleneksel ve migrenli aura, kronik karaci\u011fer hastal\u0131\u011f\u0131, ven\u00f6z tromboembolizm, malignite gibi 10 nontradisyonel risk fakt\u00f6r\u00fc incelendi. Ayr\u0131ca kad\u0131nlara \u00f6zg\u00fc gebelik komplikasyonlar\u0131 gibi 5 ek risk fakt\u00f6r\u00fc de \u00e7al\u0131\u015fmaya dahil edildi.<\/p>\n<p>\u00c7al\u0131\u015fman\u0131n en dikkat \u00e7ekici sonucu, PFO ta\u015f\u0131yanlarda nontradisyonel risk fakt\u00f6rlerin \u00f6n plana \u00e7\u0131kmas\u0131 oldu. PFO\u2019lu bireylerde klasik risk fakt\u00f6rlerinin inme riskini sadece %18 oran\u0131nda art\u0131rd\u0131\u011f\u0131 g\u00f6r\u00fcl\u00fcrken, nontradisyonel fakt\u00f6rlerin riski iki kat\u0131ndan fazla y\u00fckseltti\u011fi saptand\u0131. Buna kar\u015f\u0131n PFO bulunmayanlarda geleneksel riskler %65 oran\u0131nda bask\u0131nken, nontradisyonel fakt\u00f6rlerin pay\u0131 %27\u2019de kald\u0131. Bu durum tedavi protokollerinin PFO varl\u0131\u011f\u0131na g\u00f6re farkl\u0131la\u015ft\u0131r\u0131lmas\u0131 gerekti\u011fine i\u015faret ediyor.<\/p>\n<p>Migrenle birlikte aura belirtileri gen\u00e7 ya\u015fta kriptogenik inmelerin en g\u00fc\u00e7l\u00fc nontradisyonel risk fakt\u00f6r\u00fc olarak \u00f6ne \u00e7\u0131kt\u0131. PFO varl\u0131\u011f\u0131nda bu etkenin toplam inme riskine katk\u0131s\u0131 %50\u2019ye yakla\u015f\u0131rken, kalp defekti olmayanlarda bu oran daha d\u00fc\u015f\u00fck kald\u0131. Sa\u011fdan sola embol ak\u0131\u015f\u0131 teorisiyle uyumlu bu bulgu, migren hastalar\u0131nda PFO\u2019nun inme riskini mekanik olarak nas\u0131l art\u0131rd\u0131\u011f\u0131n\u0131 netle\u015ftiriyor. Bu nedenle migrenle birlikte aura ya\u015fayan gen\u00e7 hastalarda PFO taramas\u0131 \u00f6nem kazan\u0131yor.<\/p>\n<p>Kad\u0131nlara \u00f6zg\u00fc risklerin etkisi ara\u015ft\u0131rmada fazlaca dikkat \u00e7ekti. Gebelik komplikasyonlar\u0131 ve gestasyonel diyabet gibi \u00e7e\u015fitlendirilmi\u015f kad\u0131n riskleri, di\u011fer fakt\u00f6rlerden ba\u011f\u0131ms\u0131z olarak inme olas\u0131l\u0131\u011f\u0131n\u0131 %70 oran\u0131nda art\u0131rd\u0131. Bu bulgu gen\u00e7 kad\u0131nlarda hormonal ve fizyolojik durumlar\u0131n inme patogenezindeki rol\u00fcn\u00fc g\u00fc\u00e7lendiriyor. Klinik pratikte kad\u0131nlarda detayl\u0131 \u00fcreme \u00f6yk\u00fcs\u00fc sorgulanmas\u0131 ve risk y\u00f6netimi kritik hale geliyor.<\/p>\n<p>Klasik damar risk fakt\u00f6rleri PFO bulunmayanlarda hala y\u00fcksek bir risk olu\u015fturuyor. Bu grupta hipertansiyon, sigara, obezite gibi fakt\u00f6rler her ilave ekleni\u015fte %41 oran\u0131nda inme riskini art\u0131r\u0131yor. Ancak PFO varl\u0131\u011f\u0131nda bu etkilerin zay\u0131flamas\u0131, risk de\u011ferlendirme modellerinin homojen olmad\u0131\u011f\u0131na i\u015faret ediyor. Dolay\u0131s\u0131yla gen\u00e7lerde inme profilini belirlerken kalp anomalilerinin g\u00f6z \u00f6n\u00fcne al\u0131nmas\u0131 gerekiyor.<\/p>\n<p>\u00c7al\u0131\u015fman\u0131n tasar\u0131m\u0131 retrospektif olmas\u0131 ve bilgilerde \u00f6z bildirim y\u00f6ntemi kullan\u0131lmas\u0131 baz\u0131 k\u0131s\u0131tlamalar i\u00e7eriyor. Migren ve di\u011fer hastal\u0131klar\u0131n tan\u0131 ve raporlanmas\u0131ndaki olas\u0131 yanl\u0131l\u0131klar riski etkileyebiliyor. Bu durum; n\u00f6rolog, kardiyolog ve birincil sa\u011fl\u0131k personelinin ortak \u00e7al\u0131\u015fmas\u0131n\u0131 zorunlu k\u0131larak, daha kapsaml\u0131 bir elektronik sa\u011fl\u0131k takip sistemi ve hasta bazl\u0131 risk modellemesi gerektirir. B\u00f6ylece, PFO gibi anormaliyete sahip bireylerde migren ve di\u011fer nontradisyonel fakt\u00f6rlere y\u00f6nelik erken m\u00fcdahaleler m\u00fcmk\u00fcn olabilir.<\/p>\n<p>Son y\u0131llarda inme \u00f6nleme programlar\u0131n\u0131n temelinde klasik risk fakt\u00f6rlerinin y\u00f6netimi yer al\u0131rken, SECRETO gibi kapsaml\u0131 \u00e7al\u0131\u015fmalar bu paradigmay\u0131 de\u011fi\u015ftirmeye aday. Migrenle aura ve PFO birlikteli\u011finin embolik inme mekanizmas\u0131 \u00fczerindeki etkisi, hastalar\u0131n bireysel bazda risklerine uyarlanm\u0131\u015f tedavi yakla\u015f\u0131mlar\u0131n\u0131n geli\u015ftirilmesini te\u015fvik ediyor.
