Yeni yay\u0131mlanan bir \u00e7al\u0131\u015fmada, sabit fokal ^18F-FDG tutulumu g\u00f6r\u00fclen kolorektal b\u00f6lgedeki lezyonlarda, \u00e7ift zamanl\u0131 PET\/BT taramas\u0131n\u0131n tan\u0131sal etkinli\u011fi detayl\u0131 \u015fekilde incelendi. Hastal\u0131kla m\u00fccadelede erken ve do\u011fru tan\u0131, hasta ya\u015fam s\u00fcresi ve kalitesi ad\u0131na kritik \u00f6nem ta\u015f\u0131yor. Ancak \u00f6zellikle sabit fokal ^18F-FDG tutulumu olarak tan\u0131mlanan PET\/BT bulgular\u0131, benign ve malign olgular aras\u0131nda ayr\u0131m yapmay\u0131 g\u00fc\u00e7le\u015ftiriyor. Bu ikilem, gereksiz invaziv giri\u015fimler ya da tan\u0131daki gecikmelere yol a\u00e7abiliyor. Yeni \u00e7al\u0131\u015fma bu tan\u0131sal gri alan\u0131 netle\u015ftirmek i\u00e7in retrospektif olarak 122 hastay\u0131 kaps\u0131yor ve 141 sabit fokal tutuluma sahip kolorektal lezyonun de\u011ferlendirmesini i\u00e7eriyor.<\/p>\n<p>Ara\u015ft\u0131rma i\u00e7in se\u00e7ilen hastalarda, PET\/BT taramas\u0131n\u0131 takiben bir ay i\u00e7erisinde kolonoskopi yap\u0131larak histopatolojik do\u011frulama sa\u011flan\u0131yor. Bu kritik a\u015fama, tan\u0131 kesinli\u011fi i\u00e7in yasal temel olu\u015fturuyor. \u0130leri adenoma kavram\u0131 ise 10 mm\u2019den b\u00fcy\u00fck, vill\u00f6z yap\u0131 ve y\u00fcksek dereceli displazi gibi sitolojik \u00f6zellikler i\u00e7eren prekanser\u00f6z lezyonlar olarak tan\u0131mlan\u0131yor. B\u00f6ylece, kolon polipleri ve kanser \u00f6ncesi t\u00fcm\u00f6rler aras\u0131nda net bir ayr\u0131m yap\u0131l\u0131yor.<\/p>\n<p>\u00c7al\u0131\u015fmada, PET\/BT g\u00f6r\u00fcnt\u00fclerinden elde edilen maksimum standart uptake de\u011feri (SUVmax) ve tutulum indeksi (RI) gibi kantitatif parametreler kullan\u0131ld\u0131. SUVmax, bir lezyondaki maksimum radyon\u00fcklit birikimini yans\u0131tarak metabolik aktivitenin \u015fiddetini g\u00f6sterirken, RI erken ve ge\u00e7 d\u00f6nem g\u00f6r\u00fcnt\u00fcler aras\u0131ndaki SUVmax de\u011fi\u015fimini hesaplayarak metabolitlerin doku i\u00e7inde kal\u0131c\u0131l\u0131\u011f\u0131n\u0131 analiz ediyor. Bu dinamik de\u011ferlendirme, malign ve benign lezyonlar\u0131n ay\u0131rt edilmesinde \u00f6nemli sinyaller veriyor.<\/p>\n<p>\u0130statistiksel analizler, kolorektal kanserler ve ileri adenomalar\u0131n ge\u00e7 d\u00f6nem PET taramalar\u0131nda belirgin \u015fekilde daha y\u00fcksek SUVmax de\u011ferlerine sahip oldu\u011funu ortaya koydu (ortalama 25.1 \u00b1 14.2 vs. 14.5 \u00b1 7.5). Benzer \u015fekilde, RI de\u011ferleri malign veya pre-malign lezyonlarda \u00f6nemli oranda y\u00fcksek bulunarak, bu lezyonlar\u0131n zaman i\u00e7inde metabolit biriktirme e\u011filiminde olduklar\u0131 g\u00f6sterildi. Bu parametrelerdeki k\u00fc\u00e7\u00fck art\u0131\u015flar\u0131n bile malignite riskini anlaml\u0131 \u00f6l\u00e7\u00fcde y\u00fckseltti\u011fi tespit edildi. Her iki de\u011fi\u015fkenin birlikte kullan\u0131lmas\u0131, Al\u0131c\u0131 \u0130\u015fletim Karakteristi\u011fi (ROC) e\u011frisi alt\u0131nda kalan alan (AUC) de\u011ferini 0.801\u2019e \u00e7\u0131kararak m\u00fckemmele yak\u0131n ay\u0131r\u0131c\u0131 g\u00fc\u00e7 sa\u011flad\u0131.<\/p>\n<p>\u00c7al\u0131\u015fmada \u00f6nerilen risk s\u0131n\u0131fland\u0131rma modeli ise gecikmeli SUVmax ve RI e\u015fik de\u011ferleri esas al\u0131narak hastalar\u0131 d\u00fc\u015f\u00fck, orta ve y\u00fcksek riskli gruplara ay\u0131rd\u0131. Bu s\u0131n\u0131fland\u0131rma, ileri lezyonlar\u0131n olas\u0131 varl\u0131\u011f\u0131n\u0131 tahmin etmede \u00f6nemli klinik rehber niteli\u011finde oldu.
