Brezilya merkezli Instituto D\u2019Or de Pesquisa e Ensino ile Rede D\u2019Or ara\u015ft\u0131rmac\u0131lar\u0131n\u0131n yapt\u0131\u011f\u0131 bu \u00f6nc\u00fc \u00e7al\u0131\u015fma, insan papilloma vir\u00fcs\u00fc (HPV) ili\u015fkili lezyonlarda, ba\u011f\u0131\u015f\u0131kl\u0131k yan\u0131t\u0131n\u0131 d\u00fczenleyen programlanm\u0131\u015f \u00f6l\u00fcm-ligand\u0131 1 (PD-L1) ve forkhead box P3 (FOXP3) proteinlerinin doku \u00f6rneklerinde nas\u0131l ifade edildi\u011fini ayr\u0131nt\u0131l\u0131 olarak inceledi. Elde edilen bulgular, erken d\u00f6nemde ortaya \u00e7\u0131kan ba\u011f\u0131\u015f\u0131kl\u0131k modifikasyonlar\u0131n\u0131n, lezyonlar\u0131n malign seyri \u00fczerindeki belirleyici etkisini ortaya koyarak, invaziv kansere d\u00f6n\u00fc\u015f\u00fcm \u00f6ncesi m\u00fcdahale f\u0131rsatlar\u0131n\u0131 i\u015faret ediyor.<\/p>\n<p>HPV enfeksiyonu, anogenital kanserlerin temel nedenlerinden biri olarak kabul edilmektedir ve kal\u0131c\u0131 epitel de\u011fi\u015fikliklerine yol a\u00e7an skuam\u00f6z intraepitelyal lezyonlar\u0131n olu\u015fumunu tetikler. Bu \u00e7al\u0131\u015fma, imm\u00fcns\u00fcpresif etki g\u00f6steren FOXP3+ d\u00fczenleyici T h\u00fccreleri (Treg) ile PD-L1 ekspresyonuna odaklanarak, y\u00fcksek dereceli lezyonlarda ba\u011f\u0131\u015f\u0131kl\u0131k yan\u0131t\u0131n\u0131n bask\u0131lanmas\u0131na dair \u00f6nemli ipu\u00e7lar\u0131 verdi.<\/p>\n<p>Ara\u015ft\u0131rmac\u0131lar, anal, vulva ve penil b\u00f6lgelerden toplanan 157 hastaya ait doku \u00f6rneklerini in situ imm\u00fcnohistokimyasal y\u00f6ntemlerle de\u011ferlendirdi. Bu hastalar aras\u0131nda erkek hasta say\u0131s\u0131 95 iken kad\u0131n hasta say\u0131s\u0131 55 olarak kaydedildi. Yap\u0131lan analizlerde, HSIL\u2019lerde, LSIL\u2019lere k\u0131yasla FOXP3+ Treg h\u00fccrelerinde dikkate de\u011fer bir art\u0131\u015f g\u00f6zlendi. Treg h\u00fccrelerinin, etkili ba\u011f\u0131\u015f\u0131kl\u0131k tepkilerini bask\u0131layarak HPV ile enfekte h\u00fccrelerin ba\u011f\u0131\u015f\u0131kl\u0131ktan ka\u00e7mas\u0131na olanak sa\u011flad\u0131\u011f\u0131 bilinmektedir. Ayr\u0131ca, SP142 ve 22C3 antikor klonlar\u0131 kullan\u0131larak tespit edilen PD-L1 ifadesinin, HSIL\u2019lerde inflamatuvar ba\u011f\u0131\u015f\u0131kl\u0131k h\u00fccrelerinde \u00f6nemli \u00f6l\u00e7\u00fcde y\u00fckseldi\u011fi ortaya kondu. Bu durum, lezyon b\u00f6lgesinde aktif bir imm\u00fcn kontrol noktas\u0131 arac\u0131l\u0131ksal inhibisyon mekanizmas\u0131n\u0131n i\u015fledi\u011fini g\u00f6stermektedir.<\/p>\n<p>HSIL\u2019lerdeki PD-L1 fazlal\u0131\u011f\u0131, viral molek\u00fcllerin konak ba\u011f\u0131\u015f\u0131kl\u0131\u011f\u0131n\u0131 bask\u0131lamak i\u00e7in erken d\u00f6nemde uygulad\u0131\u011f\u0131 stratejilerden biri olarak yorumlanabilir. PD-L1\u2019in sitotoksik T h\u00fccrelerinde bulunan PD-1 ile etkile\u015fimi, T h\u00fccrelerinin fonksiyonlar\u0131n\u0131n azalmas\u0131na ve b\u00f6ylelikle antiviral ve antitumoral yan\u0131tlar\u0131n etkinli\u011finin d\u00fc\u015fmesine yol a\u00e7ar. Bu molek\u00fcler etkile\u015fim, HPV enfekte h\u00fccrelerin ba\u011f\u0131\u015f\u0131kl\u0131k sisteminden korunarak uzun s\u00fcre hayatta kalmas\u0131na ve kanserle\u015fme olas\u0131l\u0131\u011f\u0131n\u0131 art\u0131rmas\u0131na zemin haz\u0131rlar. FOXP3+ Treg h\u00fccrelerindeki art\u0131\u015f ile PD-L1 ekspresyonunun e\u015f zamanl\u0131 varl\u0131\u011f\u0131, HSIL i\u00e7indeki imm\u00fcns\u00fcpresif mikromilj\u00f6 olu\u015fturdu\u011funu ve viral persistans\u0131 destekledi\u011fini g\u00f6stermektedir.<\/p>\n<p>\u0130statistiksel analizler, bu molek\u00fcler farkl\u0131l\u0131klar\u0131n anlaml\u0131 oldu\u011funu g\u00fc\u00e7l\u00fc \u015fekilde do\u011frulad\u0131. Poisson genelle\u015ftirilmi\u015f do\u011frusal modelleme sonu\u00e7lar\u0131, HSIL\u2019lerde yo\u011fun inflamatuvar h\u00fccre infiltrasyonu frekans\u0131n\u0131n p=0.04, FOXP3+ h\u00fccre say\u0131s\u0131n\u0131n p=0.02 ve PD-L1 ekspresyonunun ise her iki antikor klonuyla da p&lt;0.01 de\u011ferleriyle anlaml\u0131 \u015fekilde y\u00fcksek oldu\u011funu ortaya koydu. Bu bulgular, imm\u00fcn bask\u0131lanman\u0131n sadece rastlant\u0131sal bir durum olmad\u0131\u011f\u0131n\u0131, HSIL\u2019lerin karakteristik bir fenotipi olarak yer ald\u0131\u011f\u0131n\u0131 desteklemektedir.<\/p>\n<p>Dikkat \u00e7ekici bi\u00e7imde bu imm\u00fcnolojik \u00f6zellikler, hastalar\u0131n HIV stat\u00fclerinden ba\u011f\u0131ms\u0131z olarak g\u00f6zlemlendi. Bu durum, HPV\u2019nin konak ba\u011f\u0131\u015f\u0131kl\u0131k sistemini manip\u00fcle etme kapasitesinin yaln\u0131zca immunosuprese hastalarla s\u0131n\u0131rl\u0131 olmad\u0131\u011f\u0131n\u0131, geni\u015f hasta kitlelerinde ge\u00e7erli oldu\u011funu g\u00f6stermektedir. B\u00f6ylece HPV\u2019nin imm\u00fcn ka\u00e7a\u011f\u0131 evrensel bir biyolojik fenomen olarak v\u00fccut bulmaktad\u0131r.<\/p>\n<p>Klinik a\u00e7\u0131dan, bu \u00e7al\u0131\u015fma \u00f6nemli \u00f6neriler sunmaktad\u0131r. FOXP3 ve PD-L1\u2019in e\u015f zamanl\u0131 y\u00fcksek ekspresyonu, lezyonlar\u0131n kal\u0131c\u0131l\u0131\u011f\u0131 ve potansiyel malign d\u00f6n\u00fc\u015f\u00fcm\u00fcn\u00fcn erken g\u00f6stergeleri olarak kullan\u0131labilir. G\u00fcn\u00fcm\u00fczde tarama ve takip stratejileri \u00f6ncelikle histopatolojik de\u011ferlendirmelere dayand\u0131r\u0131l\u0131rken, imm\u00fcnolojik belirte\u00e7lerin eklenmesi risk s\u0131n\u0131fland\u0131rmas\u0131nda daha hassas ve ki\u015fiselle\u015ftirilmi\u015f yakla\u015f\u0131mlar\u0131n geli\u015ftirilmesine katk\u0131 sa\u011flayacakt\u0131r. B\u00f6ylece y\u00fcksek risk ta\u015f\u0131yan hastalar\u0131n hedefe y\u00f6nelik izlem ve tedavi imkanlar\u0131 art\u0131r\u0131l\u0131rken, gereksiz giri\u015fimler de azalt\u0131labilir.<\/p>\n<p>Tedavi perspektifinde ise elde edilen veriler, ba\u011f\u0131\u015f\u0131kl\u0131k kontrol noktas\u0131 inhibit\u00f6rleri ve Treg d\u00fczenleyici ajanlar\u0131n premalign durumlarda kullan\u0131m\u0131n\u0131 ara\u015ft\u0131rmak i\u00e7in g\u00fc\u00e7l\u00fc bir dayanak olu\u015fturuyor. G\u00fcn\u00fcm\u00fczde lenfosit inhibit\u00f6rlerini hedef alan imm\u00fcnoterapiler ileri evre kanserlerde ba\u015far\u0131yla kullan\u0131lsa da, y\u00fcksek dereceli prek\u00fcrs\u00f6r lezyonlar\u0131n ilerleyi\u015fini engellemek \u00fczere erken evrede bu stratejilerin uygulanmas\u0131 hen\u00fcz deneysel a\u015famadad\u0131r. PD-L1 sinyal yolunun bask\u0131lanmas\u0131, antiviral ba\u011f\u0131\u015f\u0131kl\u0131\u011f\u0131 yeniden harekete ge\u00e7irip malignizasyonun \u00f6nlenmesini m\u00fcmk\u00fcn k\u0131labilir.<\/p>\n<p>Bu ara\u015ft\u0131rma HPV kaynakl\u0131 kanserle\u015fmenin ba\u011f\u0131\u015f\u0131kl\u0131k sisteminin karma\u015f\u0131k d\u00fczenlenmesi \u00fczerinden nas\u0131l seyretti\u011fine dair kavray\u0131\u015f\u0131m\u0131z\u0131 derinle\u015ftirir. Viral onkogenez, kona\u011f\u0131n ba\u011f\u0131\u015f\u0131kl\u0131k d\u00fczenlerini kendi lehine yeniden yap\u0131land\u0131rarak viral enfeksiyonun kronikle\u015fmesini ve kanser geli\u015fimini kolayla\u015ft\u0131r\u0131r. HPV\u2019nin ba\u011f\u0131\u015f\u0131kl\u0131k sistemine kar\u015f\u0131 kurdu\u011fu bu denge, imm\u00fcn aktivitenin bask\u0131lanmas\u0131 lehine bozulur; bu da PD-L1 ve FOXP3 pozitif h\u00fccrelerin bir arada bulundu\u011fu ortamda net bi\u00e7imde g\u00f6zlemlenebilir.<\/p>\n<p>Her ne kadar \u00e7al\u0131\u015fma a\u011f\u0131rl\u0131kl\u0131 olarak anogenital b\u00f6lgeye odaklanm\u0131\u015f olsa da, benzer mekanizmalar\u0131n HPV ile ili\u015fkili di\u011fer kanser t\u00fcrleri, \u00f6zellikle orofaringeal kanserlerde de ge\u00e7erli olabilece\u011fi d\u00fc\u015f\u00fcn\u00fclmektedir.
