Bu kanser t\u00fcr\u00fcn\u00fcn alt tipleri aras\u0131nda en yayg\u0131n olan\u0131 a\u00e7\u0131k h\u00fccreli renal h\u00fccreli karsinom (ccRCC) olarak bilinirken, a\u00e7\u0131k h\u00fccre d\u0131\u015f\u0131 renal h\u00fccreli karsinomlar (non-ccRCC) daha az g\u00f6r\u00fclmekle beraber farkl\u0131 biyolojik \u00f6zelliklere ve prognozlara sahiptir. Son y\u0131llarda genetik belirleyiciler \u00fczerine yo\u011funla\u015fan \u00e7al\u0131\u015fmalar\u0131n aksine, t\u00fcm\u00f6r mikro\u00e7evresindeki perirenal ya\u011f dokusunun rol\u00fc g\u00f6rece az incelenmi\u015fti. Ancak BMC Cancer\u2019da yay\u0131mlanan yeni bir ara\u015ft\u0131rma, b\u00f6brek kanserlerinin farkl\u0131 patolojik tipleri ile \u00e7evresindeki perirenal ya\u011f alan\u0131 (PFA) aras\u0131ndaki ili\u015fkiyi derinlemesine ele alarak \u00f6nemli \u0131\u015f\u0131k tuttu.<\/p>\n<p>\u00c7al\u0131\u015fma kapsam\u0131nda toplamda 297 renal h\u00fccreli karsinom hastas\u0131 retrospektif olarak de\u011ferlendirildi. Hastalar patolojik tan\u0131ya g\u00f6re iki gruba ayr\u0131ld\u0131: 236\u2019s\u0131 a\u00e7\u0131k h\u00fccreli RCC (ccRCC) ve 61\u2019i ise non-ccRCC gruplar\u0131na dahil oldu. Ara\u015ft\u0131rmac\u0131lar, b\u00f6bre\u011fin renal ven d\u00fczeyindeki kesitsel ya\u011f dokusu miktar\u0131n\u0131 \u00f6l\u00e7mek i\u00e7in bilgisayarl\u0131 tomografi (BT) g\u00f6r\u00fcnt\u00fcleme y\u00f6ntemini kulland\u0131. Bu sayede, t\u00fcm\u00f6r \u00e7evresindeki ya\u011f dokusunun hacmi ve da\u011f\u0131l\u0131m\u0131 hassas bi\u00e7imde tespit edilerek, t\u00fcm\u00f6r \u00f6zellikleri ile ba\u011flant\u0131lar kuruldu.<\/p>\n<p>Analizlerde dikkat \u00e7eken sonu\u00e7lardan biri, ccRCC hastalar\u0131n\u0131n non-ccRCC\u2019ye k\u0131yasla anlaml\u0131 derecede daha fazla perirenal ya\u011f alan\u0131na sahip oldu\u011funun ortaya \u00e7\u0131kmas\u0131yd\u0131. Bu fark sadece ya\u011f alan\u0131nda de\u011fil, ayn\u0131 zamanda beden a\u011f\u0131rl\u0131\u011f\u0131 ve beden kitle indeksi (BK\u0130) gibi sistemik adiposite ile ilgili klinik de\u011fi\u015fkenlerde de kendini g\u00f6sterdi. Bu durum, \u00e7evresel ya\u011f dokusunun sadece pasif bir depo de\u011fil, t\u00fcm\u00f6r biyolojisini do\u011frudan etkileyen aktif bir unsur oldu\u011funu vurgulad\u0131. \u00d6zellikle a\u00e7\u0131k h\u00fccreli tip i\u00e7in bu ya\u011f dokusunun \u00f6nemi artt\u0131.<\/p>\n<p>T\u00fcm\u00f6r\u00fcn b\u00f6brek i\u00e7indeki konumuna da bak\u0131ld\u0131\u011f\u0131nda, ccRCC riskinin perirenal ya\u011f alan\u0131ndaki art\u0131\u015fla \u00f6zellikle b\u00f6bre\u011fin kutup d\u0131\u015f\u0131 b\u00f6lgelerinde (OPLK) bulunan t\u00fcm\u00f6rler i\u00e7in anlaml\u0131 \u015fekilde y\u00fckseldi\u011fi g\u00f6r\u00fcld\u00fc. Her 1 cm\u00b2 artan perirenal ya\u011f alan\u0131n\u0131n, t\u00fcm\u00f6r\u00fcn a\u00e7\u0131k h\u00fccreli tip olarak ortaya \u00e7\u0131kma ihtimalini %5 oran\u0131nda art\u0131rd\u0131\u011f\u0131 tespit edildi. Bu mek\u00e2nsal ili\u015fki, ya\u011f dokusunun t\u00fcm\u00f6r geli\u015fimi \u00fczerindeki etkisinin anatomik konumdan etkilenebilece\u011fini g\u00f6sterdi.<\/p>\n<p>Ayr\u0131ca, erken evre t\u00fcm\u00f6rler (patolojik T1 evresi) incelendi\u011finde, b\u00f6bre\u011fin orta b\u00f6lgesinde yer alan t\u00fcm\u00f6rlerde perirenal ya\u011f alan\u0131n\u0131n ccRCC riskini %6 oran\u0131nda art\u0131rd\u0131\u011f\u0131 kaydedildi. Erken evre t\u00fcm\u00f6rlerde bu ili\u015fkinin daha belirgin olmas\u0131, tan\u0131 ve tedavi planlamas\u0131nda perirenal ya\u011f alan\u0131n\u0131n potansiyel bir biyobelirte\u00e7 olarak kullan\u0131labilece\u011fini i\u015faret etmektedir. \u00d6zellikle cerrahi kararlar\u0131n al\u0131nmas\u0131nda fayda sa\u011flayabilir.<\/p>\n<p>Ara\u015ft\u0131rman\u0131n biyolojik alt yap\u0131s\u0131na bak\u0131ld\u0131\u011f\u0131nda, perirenal ya\u011f dokusunun sadece ya\u011f depolama i\u015flevinin \u00f6tesinde; adipokinler, sitokinler ve b\u00fcy\u00fcme fakt\u00f6rleri gibi pek \u00e7ok biyolojik aktif molek\u00fcl salg\u0131layarak t\u00fcm\u00f6r mikro\u00e7evresinin inflamasyon ve h\u00fccre sinyalizasyon yollar\u0131n\u0131 etkileyebilece\u011fi \u00f6ne s\u00fcr\u00fcld\u00fc. Bu durum ccRCC olu\u015fumunda ve ilerlemesinde perirenal ya\u011f dokusunun rol\u00fcn\u00fc g\u00fc\u00e7lendirmektedir. T\u00fcm\u00f6r ve \u00e7evresindeki ya\u011f dokusu aras\u0131nda kompleks bir biyokimyasal ileti\u015fim mevcuttur.<\/p>\n<p>\u00c7al\u0131\u015fmada ayr\u0131ca, patolojik evrelere g\u00f6re perirenal ya\u011f alan\u0131ndaki farkl\u0131l\u0131klar\u0131n ccRCC ve non-ccRCC gruplar\u0131nda de\u011fi\u015fiklik g\u00f6sterdi\u011fi ortaya kondu. Bu da ya\u011f dokusunun sadece t\u00fcm\u00f6run ortaya \u00e7\u0131k\u0131\u015f\u0131nda de\u011fil, ayn\u0131 zamanda ilerlemesinde ve agresifli\u011finde de etkili olabilece\u011fini d\u00fc\u015f\u00fcnd\u00fcrmektedir.
